Perennials Plus Wholesale Credit Terms

The Applicant has delivered this request to John Platte Nursery Sales, Inc. DBA//Perennials Plus for the extension of credit. The applicant swears that the information received is accurate to the best of his/her knowledge and understands that Perennials Plus will rely on the accuracy and completeness of this statement. Applicant must be 18 years old.

I/We authorize John Platte Nursery Sales, Inc. DBA/Perennials Plus to obtain credit information from the above listed credit references.

I/We agree to pay the total amount due by the last day of the month following the statement date.

I/We understand that if the full balance is not paid when due, a monthly finance charge of two percent (2%) which is equivalent to twenty-four percent (24%) APR will be assessed and applied to the current balance. A minimum finance charge of $.50 applies.

I/We understand that a convenience charge of three percent (3%) may be applied to any account balance paid by way of a credit card.

I/We understand that if the account becomes delinquent by 30 days past due, finance charges will be paid first and payment applied to the oldest invoice and in that order only.

I/We understand that if the account becomes delinquent by 60 days past due, credit privileges may be temporarly revoked, and COD only accepted.

I/We understand that if the account becomes delinquent by 90 days past due, credit privileges may be revoked for one year, and COD only accepted. 

I/We also agree to pay all fees assessed on the account for collection of the balance without the relief from valuation and appraisement laws. Fees may include, but not limited to, all attorney fees and court cost.

I/We acknowledge that failure to sign this guarantee of payment will give John Platte Nursery Sales, Inc. DBA/Perennials Plus the right to refuse credit.

I/We also agree to pay a returned check fee of $35.00, if applicable.
